Kingston Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team at Kingston Health Center of Vermilion in Vermilion, OH. In this role, you will provide high-quality nursing care to residents in our assisted living, rehabilitation, and memory care programs. Working under the direction of a Registered Nurse and in coordination with an interdisciplinary care team, you will deliver hands-on clinical care, monitor resident conditions, and support individualized care plans that enrich our residents’ quality of life. As an LPN at Kingston Health Center, you will perform routine nursing procedures such as administering medications, taking vital signs, performing basic wound care, assisting with treatments and therapies, and documenting all care accurately in electronic medical records. You will observe residents for changes in condition, promptly report findings to the RN or provider, and help coordinate appropriate interventions. You will collaborate closely with physicians, therapists, social workers, and other healthcare professionals to ensure that each resident’s physical, emotional, and social needs are met. You will also provide direct support with activities of daily living as needed, promote resident safety and comfort, and offer compassionate interaction to residents and their families, especially those in rehabilitation and memory care settings. This includes following care plans, using de‑escalation and redirection techniques for residents with cognitive impairments, and maintaining residents’ dignity and independence. Strong communication skills are essential as you will explain care procedures, provide updates to families, and participate in care conferences.
