Sanford Health is seeking a dedicated RN, Minimum Data Set (MDS), to support high-quality resident care and regulatory compliance within our Healthcare & Medical Services department. As an MDS RN, you will be responsible for coordinating and completing comprehensive resident assessments, ensuring that all Minimum Data Set (MDS) documentation is accurate, timely, and compliant with federal and state regulations. You will collaborate closely with interdisciplinary team members, including physicians, nurses, therapists, social workers, and case managers, to gather clinical information, reflect residents’ true clinical status, and support individualized care plans. Drawing on your clinical expertise, you will review medical records, conduct resident interviews and assessments, and translate clinical findings into accurate MDS entries that drive case-mix reimbursement, quality measures, and care outcomes. In this role, you will monitor assessment schedules, track due dates, and ensure that all OBRA and PPS assessments are completed within required time frames. You will help analyze quality indicator and quality measure reports, identify trends and variances, and work with clinical leaders to implement performance improvement activities. As a resource for the care team, you will provide education on MDS requirements, documentation standards, and best practices for accurate charting. You will also participate in care conferences, contribute to person-centered care planning, and support discharge planning to promote safe and effective transitions of care. The MDS RN will help prepare for regulatory surveys and payer audits, maintain up-to-date knowledge of changing MDS guidelines, and support policy and procedure updates as needed. This position is ideal for an experienced registered nurse who is detail-oriented, highly organized, and comfortable working with electronic health records and clinical data.
