The Bariatric Dietitian Case Coordinator will manage a dedicated caseload of bariatric patients, providing individualized medical nutrition therapy, education, and ongoing follow-up across the continuum of care. You will conduct detailed nutrition assessments, develop personalized meal and supplementation plans, and closely monitor nutritional status, weight trends, and tolerance to dietary progression following surgery. Using current bariatric and obesity medicine guidelines, you will help patients prepare for surgery, understand required lifestyle changes, and maintain adherence to post-operative protocols.
Working within a multidisciplinary team, you will collaborate daily with bariatric surgeons, advanced practice providers, nurses, behavioral health professionals, and program coordinators to align care plans and support a seamless patient experience. You will participate in case conferences, communicate changes in patients’ nutritional or clinical status, and contribute to shared decision-making regarding readiness for surgery and long-term follow-up needs. Accurate and timely documentation in the electronic health record will be essential, including assessments, education provided, goals, progress notes, and outcomes.
Patient education and engagement are central to this role. The Bariatric Dietitian Case Coordinator will lead and facilitate individual counseling sessions as well as group classes, orientation sessions, and follow-up education visits. Using motivational interviewing and other behavior-change strategies, you will guide patients through complex dietary changes such as pre-operative weight loss goals, staged diet advancement after surgery, supplementation protocols, and long-term nutrient-dense eating patterns. You will help patients recognize and address potential issues such as dumping syndrome, food intolerances, inadequate protein or fluid intake, and micronutrient deficiencies.
Case coordination responsibilities include tracking patients through the bariatric care pathway, from initial information sessions through evaluation, surgery, and long-term follow-up. You will monitor appointment adherence, proactively follow up with patients who miss or reschedule visits, and coordinate with the care team to reduce barriers to successful participation. You will support program quality initiatives by collecting and monitoring relevant data, identifying trends in patient outcomes, and helping implement process improvements aligned with Carle Health’s commitment to safety, quality, and innovation.
The ideal candidate is a Registered Dietitian Nutritionist with strong clinical skills, excellent communication abilities, and a passion for helping patients navigate significant lifestyle change. Experience in bariatric nutrition, weight management, endocrinology, or related clinical areas is highly valued. Comfort with technology, including electronic health records and virtual visit platforms, is important as some patient encounters may occur via telehealth. You should be able to manage a dynamic caseload, prioritize tasks efficiently, and collaborate effectively in a fast-paced, patient-centered environment.
