As a Physical Therapist Assistant with VNS Health, you will help deliver high-quality, patient-centered rehabilitative care to individuals in their homes and community settings throughout New York City. Working under the direction and supervision of a licensed Physical Therapist, you will carry out individualized treatment plans designed to improve patients’ mobility, strength, balance, and functional independence, while promoting safety and quality of life. In this role, you will provide hands-on therapeutic interventions such as therapeutic exercises, gait and transfer training, balance and coordination activities, and use of assistive devices and durable medical equipment. You will observe and document patients’ responses to treatment, track progress toward functional goals, and communicate changes in condition or needs to the supervising Physical Therapist, nurses, and other members of the care team. You will also reinforce patient and caregiver education on home exercise programs, safe body mechanics, fall prevention, and proper use of equipment to support ongoing recovery between visits. You will travel to patients’ homes and community-based locations across New York City, adapting treatment approaches to varied home environments and cultural needs. Collaborating closely with an interdisciplinary team, you will participate in care coordination to support comprehensive plans of care that reflect VNS Health’s commitment to compassionate, holistic, and evidence-based practice. You will use electronic medical records to complete accurate, timely clinical documentation that meets regulatory and organizational standards.
