LifeBridge Health, a regional healthcare organization in Maryland providing a comprehensive continuum of care through hospitals, specialty centers, and community health-related operations, is seeking a dedicated Clinical Pharmacist to join our Healthcare & Medical Services department. As a Clinical Pharmacist at LifeBridge Health, you will play a key role in delivering compassionate, safe, and evidence-based medication therapy to a diverse patient population while collaborating closely with physicians, nurses, and other members of the interdisciplinary healthcare team. You will be responsible for reviewing and verifying medication orders for appropriateness, dosing, interactions, and allergies; optimizing pharmacotherapy through therapeutic drug monitoring, renal and hepatic dose adjustments, and adherence to institutional protocols and clinical guidelines. You will provide direct patient care by conducting medication therapy management, counseling patients and families on proper medication use, side effects, and adherence strategies, and serving as a readily available drug information resource for clinical staff. Using our electronic health record and pharmacy information systems, you will document interventions, monitor clinical outcomes, and contribute to medication-use evaluations and quality improvement initiatives that support LifeBridge Health’s culture of continuous improvement and innovation. The role includes active participation in medication safety efforts, antimicrobial stewardship, and formulary management, as well as ensuring compliance with Joint Commission, CMS, and state Board of Pharmacy standards. You may also have opportunities to precept pharmacy students and residents, attend professional development activities, and engage in projects that improve healthcare delivery and community health.
