Matrix Medical Network is seeking a compassionate and adaptable Nurse Practitioner – PRN to join our Healthcare & Medical Services team. In this highly flexible, field-based role, you will perform comprehensive in-home health assessments, wellness visits, and focused clinical evaluations for health plan members and government program participants. Working with a diverse adult and geriatric population, you will leverage your advanced clinical training to evaluate overall health status, identify and manage chronic conditions, recognize emerging risks, and support preventive care. As a Nurse Practitioner with Matrix Medical Network, you will conduct detailed histories and physical examinations, review medications and treatment plans, order and interpret appropriate diagnostic tests within scope, and document your findings accurately in electronic health records. You will collaborate closely with the broader care team, including primary care providers, health plans, and quality management staff, to ensure that each assessment results in clear, actionable recommendations to improve patient outcomes and close care gaps. This PRN position offers the opportunity to create a schedule that complements your existing commitments while still making a meaningful impact. You will travel to members’ homes and community settings within your assigned territory, applying strong clinical judgment and independent practice skills in environments that require excellent organization, communication, and professionalism. You will educate patients and caregivers on disease management, medications, lifestyle changes, and available resources, empowering them to actively participate in their care.
