Matrix Medical Network is seeking a compassionate and skilled Physician Assistant to join our Healthcare & Medical Services team, supporting our mission to improve patient outcomes and transform how healthcare is delivered. As a key clinical member of our organization, you will provide high-quality in-home and virtual health assessments, manage acute and chronic conditions within your scope of practice, and collaborate closely with a multidisciplinary team to deliver patient-centered care. In this role, you will perform comprehensive health risk assessments, focused physical examinations, and evidence-based clinical evaluations for patients enrolled through insurance plans and government agencies. You will obtain detailed medical histories, review medications, order and interpret appropriate diagnostic tests, and develop individualized care plans in alignment with current clinical guidelines and Matrix Medical Network protocols. Your work will emphasize prevention, early identification of risk, and coordination of care with primary care providers, specialists, and care management resources. You will document all encounters accurately and efficiently in our electronic health record system, ensuring that clinical notes, coding, and quality metrics meet regulatory and organizational standards. You will educate patients and caregivers on disease processes, medications, lifestyle modifications, and community resources, helping them understand and actively participate in their care plans. This position involves regular travel within the community to conduct in-home visits, along with opportunities for telehealth-based encounters, requiring strong time management, adaptability, and comfort working independently in the field. Aligned with Matrix Medical Network’s culture of innovation and collaboration, you will participate in quality improvement initiatives, case discussions, and clinical training designed to enhance patient outcomes and reduce healthcare costs. You will work closely with nurses, physicians, care coordinators, and quality teams to identify gaps in care, close preventive screening measures, and support transitions of care. The ideal candidate is a licensed Physician Assistant with solid clinical judgment, excellent communication skills, and a passion for delivering patient-centered care outside of traditional clinic walls. You should be comfortable working with diverse adult and senior populations, often with multiple chronic conditions, and be able to build trust quickly in the home setting. Familiarity with managed care, value-based care models, or health risk assessments is a plus.
