Job Summary

The Team Coordinator is responsible for leading and managing the Community Support Team (CST). This role ensures that all staff meet training and certification requirements, that services remain compliant with regulatory standards, and that consumers receive high-quality, person-centered care.

Key Responsibilities

Team Leadership & Oversight

Identify, hire, and manage all CST team members, including:

Mental Health Therapist

Substance Abuse Specialist

Qualified Professional (Case Manager)

Peer Support Specialist

Paraprofessional

Ensure all staff maintain required certifications and complete mandatory trainings for their roles.

Facilitate weekly CST meetings and ensure in-person attendance.

Oversee timely and accurate submission of staff documentation and ensure compliance with agency and payer requirements.


Quality Assurance & Compliance

Conduct ongoing quality assurance to ensure consumer services align with service definitions.

Verify that all intakes assessments, addendums, Person-Centered Plans (PCPs), Service Orders (SARs), and DLA-20s are current and accurate.

Ensure Treatment Team (TT) meetings are held monthly with the CST team and consumer, with attendance documented and signed.

Monitor compliance for all consumers monthly, verifying services remain within authorization guidelines and also verify insurance coverage on monthly bases.

Make sure QP is linked to NC Topps and submit monthly reports on their caseload and any other reports due to MCO monthly.

Make sure RSVP are being submitted and updated once completed and followed up upon submitting


Confidentiality

Maintain strict confidentiality regarding consumer information and service-related

records in accordance with agency policies, state regulations, and HIPAA requirements.
