LifeBridge Health is seeking a dedicated Clinical Pharmacist I to join our Healthcare & Medical Services team within our Healthcare & Medical Services department. The Clinical Pharmacist I is responsible for providing safe, effective, and cost-conscious medication therapy management for a diverse patient population across inpatient and ambulatory settings. Working as an integral member of an interdisciplinary care team, you will perform prospective and retrospective clinical review of medication orders, verify and adjust therapy based on current clinical guidelines, patient-specific factors, and pharmacokinetic principles, and ensure adherence to institutional policies, formularies, and regulatory standards. You will participate in daily patient care rounds where applicable, collaborate closely with physicians, advanced practice providers, and nurses, and serve as a key resource for medication-related questions and therapeutic decision-making. Core responsibilities include reviewing and verifying medication orders for appropriateness, dosage, interactions, contraindications, and therapeutic duplication; performing medication reconciliation at key transitions of care; and monitoring the safety and effectiveness of medication regimens. You will provide direct patient counseling and education on medication indications, administration, side effects, adherence strategies, and lifestyle considerations, with particular attention to high-risk therapies and vulnerable populations. The role also involves supporting antimicrobial stewardship initiatives, managing therapeutic drug monitoring for medications requiring serum level interpretation and dose adjustment, and assisting in the development and maintenance of clinical guidelines, protocols, and order sets. The Clinical Pharmacist I will participate in sterile and non-sterile compounding processes in accordance with USP standards, ensuring accuracy, sterility, and stability of compounded preparations when needed. You will document clinical interventions, medication histories, and patient education in the electronic health record, using clinical decision support tools to enhance safety and efficiency. Working in collaboration with pharmacy leadership, you will help identify opportunities for process improvement, contribute to quality and performance initiatives, and support medication safety efforts including adverse drug event detection, reporting, and follow-up. Education and professional development are central to LifeBridge Health’s mission, and the Clinical Pharmacist I plays a role in mentoring and precepting pharmacy students and residents, as appropriate to experience. You will assist in orienting new pharmacy staff, participate in in-service education for nursing and medical staff, and remain current with emerging therapies, guidelines, and technologies through continuing education and self-directed learning. The position requires strong communication skills, critical thinking, attention to detail, and the ability to work effectively in a fast-paced, team-based environment that serves diverse communities. This full-time position may include a combination of day, evening, weekend, and holiday shifts, with the potential for limited remote clinical verification or project-based work depending on departmental needs. The role is primarily field/on-site within LifeBridge Health facilities in Maryland, with hybrid components as available.
