*Responsibilities*
Evaluate and treat patients admitted for post-acute care.
Provide direct patient care focused on functional recovery, safety, and improved quality of life.
Collaborate with a multidisciplinary team to develop personalized treatment plans
Document according to facility and CMS guidelines.
Supervise Occupational Therapy Assistants in direct patient care and patient related activities and adhere to facility Supervision Policy and Procedure Matrix
Communicate with supervisor and other health team members regarding patient progress, problems and plans
Educate patients and families on therapeutic goals and safe discharge planning
*Qualifications*
Licensed Occupational Therapist (OT).
Open to applicants of all experience levels, including new graduates.

Pay: $38.00 - $48.00 per hour
