Nurse Practitioner

Matrix Medical Network is seeking a compassionate and forward-thinking Nurse Practitioner to join our Healthcare & Medical Services team, delivering high-quality clinical care directly where patients live. As a key member of our field-based clinical staff, you will perform comprehensive health assessments, manage acute and chronic conditions, and partner with patients, families, providers, and health plans to improve outcomes and reduce avoidable healthcare costs. In this role, you will conduct in-home and community-based visits with adult and senior populations, completing detailed health risk assessments, physical examinations, and evidence-based screenings. You will identify gaps in care, update medication lists, review medical histories, and develop individualized care plans that support better disease management, safety in the home, and improved quality of life. Working both independently in the field and collaboratively with a multidisciplinary team, you will communicate findings to primary care providers and care managers, recommend appropriate follow-up, and support care coordination across settings. The Nurse Practitioner will leverage strong clinical judgment and critical thinking skills to evaluate complex patients, address acute concerns within your scope of practice, and provide patient and family education on disease processes, medications, lifestyle changes, and community resources. You will document all encounters using electronic health record systems, ensuring accuracy, clarity, and compliance with regulatory and quality standards. You will also contribute to quality assurance initiatives by adhering to clinical guidelines, participating in peer review or case discussions, and identifying opportunities to enhance care delivery.
