Mohawk Valley Health System (MVHS) is seeking a dedicated Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services department within our dynamic hospital and outpatient care settings in Upstate New York. As an integral member of an interdisciplinary healthcare team, the LPN will provide direct, patient-centered nursing care under the direction of a Registered Nurse and/or provider, supporting MVHS’s mission to improve the health of the communities we serve. In this role, the LPN delivers compassionate, evidence-based care to patients across a variety of acute and sub-acute settings, including medical-surgical units, specialty clinics, and primary care practices. Responsibilities include obtaining and documenting vital signs, performing patient assessments within the LPN scope of practice, assisting with care planning, and implementing nursing interventions. The LPN will safely administer medications and treatments as ordered, monitor patient responses, promptly report changes in condition to the RN or provider, and contribute to safe transitions of care through accurate, timely documentation in the electronic health record. The LPN will collaborate closely with RNs, physicians, advanced practice providers, therapists, and ancillary staff to ensure coordinated, high-quality care. Key duties include preparing patients for examinations and procedures, assisting providers during treatments, performing wound care and dressing changes, collecting specimens, and supporting infection prevention practices. The LPN will also provide patient and family education on disease processes, medications, discharge instructions, and health maintenance, reinforcing teaching provided by the RN and providers. The ideal candidate is a compassionate, detail-oriented clinician with strong communication and organizational skills, able to prioritize multiple tasks in a fast-paced environment. The LPN should be comfortable using electronic health records, following established clinical protocols, and escalating patient concerns appropriately. A commitment to professional growth, openness to feedback, and willingness to precept or mentor new staff and students as appropriate are highly valued.
