Physical Therapist

Human Performance and Rehabilitation Centers is seeking a dedicated and patient-focused Physical Therapist to join our Healthcare & Medical Services team in the Houston area. In this role, you will provide advanced physical therapy and rehabilitation services to a diverse caseload, including orthopedic, post-surgical, neurological, and performance-oriented patients. You will complete thorough evaluations, develop individualized treatment plans, and deliver evidence-based interventions designed to restore function, reduce pain, and enhance overall performance. Working in a supportive and collaborative environment, you will partner closely with patients, families, referring providers, and fellow clinicians to ensure patient-oriented care from initial evaluation through discharge. Your daily responsibilities will include performing detailed physical and functional assessments, setting meaningful and measurable goals, implementing manual therapy and therapeutic exercise programs, instructing patients in home exercise programs, and monitoring progress using objective outcome measures. You will document all care accurately and efficiently in our electronic medical record system, maintaining clear, timely, and compliant clinical notes. You will play an important role in patient education, helping individuals understand their condition, treatment options, and strategies for injury prevention and long-term wellness. You will also contribute to the overall performance and quality of the clinic by participating in case conferences, collaborating on complex cases, and sharing best practices with colleagues.
