We are seeking Licensed Physical Therapists to fill Home Health positions in Macomb, Oakland, Wayne, Genesee and Livingston counties.Candidate will be able to select county in which they prefer working Pre-hire.
*Job Responsibilities:*
* Provide quality home health care physical therapy services
* Evaluate and assess your patients to establish functional
* Measurable goals that promote restoration of functional abilities
* Advise and consult with the family and other caregivers as indicated to promote the client's level of progress toward mutually agreed upon goals and plan for discharge
Qualifications * Current Michigan state license to practice as a Physical Therapist
* Degree from PT Accredited College
* Minimum one (1) year of Physical Therapist (PT) experience required with home health experience preferred
* Current CPR certification
* Copy of updated TB results
* Reliable transportation
