Nurse Practitioner (Primary Care, Women’s Health Focus)
Location: Plainfield, IL

About the Role
St. Mary Internal Medicine is a growing, physician-led primary care practice seeking an experienced Nurse Practitioner to join our team.
This role is focused on delivering high-quality, relationship-driven care in an efficient outpatient setting. The ideal candidate is confident managing chronic conditions, performing annual physicals, and providing women’s health services, while working collaboratively within a small, supportive team.
You will play a key role in expanding patient access and helping scale a modern, community-based primary care practice.
Key Responsibilities
- Manage chronic conditions including hypertension, diabetes, and hyperlipidemia
- Perform annual physicals and preventive care visits
- Provide women’s health services including Pap smears, contraception counseling, and routine gynecologic care
- Evaluate and treat acute, low-to-moderate complexity conditions
- Manage medication refills and ongoing treatment plans
- Collaborate with physician on complex cases as needed
- Build and maintain long-term patient relationships
Qualifications
- Licensed Nurse Practitioner in Illinois
- Minimum 2 years of primary care experience required
- Comfortable managing patients independently in an outpatient setting
- Experience with women’s health (Pap smears, contraception, routine GYN care)
- Efficient and comfortable seeing 12–16 patients per day
- Strong clinical judgment and communication skills
