*RN Clinical Supervisor:*
Our organization is expanding into home health skilled nursing services, and we are seeking a Registered Nurse with home health experience to join our CHAP accredited team.
The Clinical Supervisor is responsible for the development of the individualized plan of care clinical oversight and delivery of a designated customer’s medical needs per the contract and care plan/physician orders (485). As we grow, the Clinical Supervisor will support the professional growth, orientation and development of the field staff while also fostering team engagement and enhancing overall performance and metrics. Critical to this role is the assessment of clinical experience, skills, and competency of caregivers. This oversight will include supervision of care teams and other duties as assigned.
In addition to this role the Clinical Supervisor acts as the Alternate Director of Nursing in the absence of the Director of Nursing. The Clinical Supervisor also acts as the Alternate Disaster Coordinator and a member of various committees.
The Clinical Supervisor must demonstrate competency in all skill sets offered to clientele: medication (PO, and IV)/treatment administration, documentation, enteral tube management (gastrostomy and/or jejunostomy), naso-gastric tube management, blood glucose monitoring and diabetic management, ketone monitoring, chest therapies (cough assist, chest therapy vest/manual chest therapy), nebulizer, CPAP/BiPAP, tracheostomy management (cuffed/uncuffed), ventilator management), wound care, foley/straight catheterization, seizure management. General pediatric and adult assessment and vital signs, focused assessment (cardiovascular, respiratory, neurological, gastrointestinal, pain, ears/nose/throat/mouth/head, genito-urinary/reproductive, endocrine, musculoskeletal, integumentary), seizure management, intake/output, communication, CPR. The clinical supervisor will demonstrate effective teaching to our clients and their family.
The Clinical Supervisor exudes positivity and will be required to maintain flexibility regarding work schedule which may include occasional patient visits on weekends, or before/after regular office hours, as well as rotating on-call responsibilities. As a new organization, duties may shift as we evolve. Our normal office hours are Monday – Friday 9:00am-5:00pm.
*Essential Duties and Responsibilities:*
· Timely visits with patients per orders and regulations
· Ensures all patient and caregiver requirements are completed per Federal, State, and local contracts and program regulation and agency policy.
· Functions as a clinical partner with respective Business Development and recruitment team
· Provides clinical oversight/supervision and case management as required by contract or regulation
· Provides and ensures client care coordination and transition management activities, as directed by contract, across the continuum
· Facilitates the orientation process and competency validation process (as required by program or contract) for caregivers
· Provides documentation support and review as required by contract and provides re-education when necessary
· Develops caregivers and conducts performance evaluations as required by agency policy, contract, and/or accrediting bodies
· Manages caregivers while on assignment (clinical functions); assesses and investigates caregiver concerns and issues, and provides performance coaching, counseling, and disciplinary action when appropriate
· Participates in clinical quality initiative activities
· Conducts clinical interviews and participates in hiring decisions in collaboration with clinical and operational leadership teams
· Will be required to be on-call for triage for clinical questions and concerns, and potentially on unscheduled visits
· Accountable to all Federal, State, local contracts and programs, and agency requirements
· Performs other duties as assigned
· Ensures all patient and caregiver requirements are completed per regulation
*Minimum Requirements:*
· Current, unencumbered Registered Nurse (RN) license required (Pennsylvania)
· Minimum one year experience as a Registered Nurse in the Home Health setting
Minimum 2 years clinical experience in the acute care setting
· Must meet all Federal, State, local contract, and program requirements, in addition to internal certifications and training, as required
· Must have ability and willingness to travel within respective service area
· Excellent written and verbal communication skills
· Demonstrated critical thinking skills, and competency with all clinical skills provided
· Supervisory experience preferred
· Experience in quality management preferred
· Proficiency with computers, including Microsoft Office
· Current PPD or chest X-ray
· Current CPR card
· Experience with OASIS preferred
· Knowledge of CMS, ODP and OLTL guidelines preferred
· Proof of COVID-19 vaccination
