*Position Summary*
We are seeking a _Speech Language Pathologist (SLP)_ to join our growing home health team. In this role, you will provide skilled speech therapy services to patients in their homes, helping them regain and improve communication and swallowing abilities.
*Key Responsibilities*
* Deliver care in accordance with state speech therapy practice acts, the patient’s plan of care, and in coordination with the interdisciplinary team.
* Evaluate patients’ functional abilities using appropriate tests and assessments.
* Develop and implement treatment plans to address communication and swallowing disorders.
* Document visit notes and patient responses during or immediately after each session.
* Educate patients, families, and caregivers on therapy techniques they can use at home.
* Collaborate with supervising RN and other team members to adjust care plans as needed.
* Participate in staff meetings, in-services, and case conferences.
* Maintain up-to-date records and communicate changes in patient status.
* Complete evaluations and care plans in accordance with state guidelines.
* Provide occasional in-services upon request.
*Qualifications*
* Current Oregon license in Speech Language Pathology (required)
* Current CPR certification (required)
* Valid driver’s license, proof of insurance, and reliable transportation (required)

Pay: Up to $105,000.00 per year
