_*We are a 240 bed facility with multiple units--Memory Care, Skilled Nursing and Rehab, and a Sub Acute! Extensive orientation and training provided for Rehab Team.*_
*San Diego Post Acute *Is looking for a compassionate, committed, and skilled *Speech Therapist *to join our team! Full-time and part-time position available.
*Purpose: *To provide the highest quality of speech therapy services to our residents/patients consistent with established standards of practice and facility procedures.
*Duties and Responsibilities:*
* Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
* Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
* Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
* Establish individual resident therapy schedules in cooperation with other team members
* Conduct therapy according to established treatment plan and ensure proper documentation and billing of therapy services
*Requirements and Qualifications:*
* Active state license as a speech-language pathologist
* Current BLS/CPR certification
* At least 1 year of proven experience as an SLP within the last 5 years
