Pine Creek Care Center is seeking a dedicated and compassionate Licensed Physical Therapist to join our interdisciplinary care team. In this full-time role, you’ll help patients regain mobility, restore function, and enhance their quality of life through personalized therapy and evidence-based interventions. Key Responsibilities:
* Patient Evaluation & Care Planning:
* Conduct comprehensive physical therapy assessments.
* Develop and implement individualized treatment plans in coordination with physician orders.
* Collaborate with referring physicians and healthcare team members to ensure continuity of care.
* Direct Treatment & Supervision:
* Provide skilled therapy sessions tailored to patient needs and goals.
* Supervise Physical Therapist Assistants, therapy aides, and students per state practice standards.
* Assist in the training and guidance of Restorative Aides in collaboration with the nursing department.
* Team Collaboration:
* Participate in care planning meetings, utilization reviews, and interdisciplinary team conferences.
* Effectively communicate patient progress, challenges, and discharge planning with the care team.
* Documentation & Compliance:
* Complete evaluations, daily notes, weekly progress reports, and recertifications using NetHealth documentation software.
* Ensure all documentation meets state, federal, and third-party payer regulations.
* Family Training & Discharge Support:
* Educate patients and caregivers on home exercise programs and safety practices.
* Recommend and help coordinate the provision of durable medical equipment (e.g., walkers, braces, grab bars).
* Equipment Oversight:
* Report equipment issues to ensure a safe and effective therapy environment.
Qualifications:
* Education:
* Bachelor’s degree in Physical Therapy required.
* Master’s or Doctorate in Physical Therapy preferred.
* Licensure:
* Current Physical Therapist license in the state of California.
* Must remain in good standing and meet continuing education requirements.
* Skills:
* Strong clinical, organizational, and communication skills.
* Experience with NetHealth (or similar EMR platforms) preferred.
* Team-oriented mindset with a passion for quality care.

* Frequent walking, standing, reaching, and lifting.
