*Physical Therapist – Skilled Nursing Facility*
We’re looking for a Physical Therapist who cares about helping people get back on their feet and regain as much independence as possible after illness, injury, or surgery.
This role is hands-on and focused on real outcomes for patients. Many of our residents are coming from the hospital or acute care facility and need help rebuilding strength, balance, and mobility so they can return home safely or improve their day-to-day function.
You’ll evaluate new admissions, determine where they are physically, and create a clear plan to help them improve. Then you’ll work with them daily to build strength, improve walking and balance, reduce fall risk, and help them move more independently.
The goal is simple: help people function better and stay safer.
What You’ll Do
* Perform *admission evaluations* for new patients to determine mobility, strength, balance, and fall risk
* Create practical therapy plans with clear goals focused on improving daily function
* Provide *daily physical therapy treatments* including gait training, transfer training, balance work, strengthening, and endurance training
* Help patients safely use walkers, wheelchairs, or other assistive devices
* Track patient progress and adjust therapy plans as they improve
* Document evaluations, treatments, and progress according to skilled therapy requirements
* Work closely with nursing staff and other therapists to coordinate patient care
* Participate in care planning and discharge planning discussions
What We’re Looking For
* Licensed Physical Therapist in the state
* Experience in skilled nursing, rehab, or geriatric care is helpful but not required
* Someone who is practical, patient-focused, and comfortable working with older adults
* Strong clinical judgment and attention to patient safety
Why This Role Matters
Many of the people we care for have just gone through a major medical event. The work you do helps them regain mobility, prevent falls, and improve their quality of life.
Sometimes that means helping someone walk again after surgery.
Sometimes it means helping them safely get out of bed or move to a chair.
Either way, your work directly improves how our residents live every day and is an important part in how their health is restored.
Job Type: Full-time
Pay: $50.00 - $55.00 per hour
Expected hours: 40 per week
