Join our dynamic healthcare team as a_*PRN Speech-Language Pathologist,*_ working in a Home Health and Hospice setting. *Duties*
* Conduct comprehensive patient assessments to identify speech, language, and swallowing disorders.
* Develop and implement personalized treatment plans aligned with each patient’s needs and goals.
* Deliver engaging speech therapy sessions focused on improving communication skills, social interaction, and feeding abilities.
* Collaborate closely with families, caregivers, and multidisciplinary teams to ensure continuity of care.
* Monitor progress regularly, adjusting therapy techniques based on patient response and physiology changes.
* Maintain detailed documentation of assessments, treatment plans, progress notes, and discharge summaries in compliance with home health and hospice standards.
* Stay current with advancements in speech therapy practices and related medical terminology to ensure the highest quality of patient care.
*Qualifications*
* Valid licensure or certification as a Speech-Language Pathologist (SLP).
* Prior experience working in a Home Health and Hospice setting preferred but not required.
* Strong knowledge of physiology, anatomy related to speech and swallowing functions, and medical terminology.
* Proven ability to perform thorough patient assessments and develop effective treatment strategies.
* Experience with delivering speech therapy services to hospice and home health patients is a plus.
* Excellent communication skills for collaborating with families and healthcare teams.
* Ability to work independently in a PRN capacity while maintaining high standards of patient care.
