We are seeking a dedicated and compassionate *Physical Therapist* to join our healthcare team. The ideal candidate will provide expert rehabilitative services to patients across various settings, including inpatient, outpatient, home health, and community care. This role involves developing personalized care plans, implementing physiotherapy techniques, and collaborating with multidisciplinary teams to promote optimal patient recovery and mobility. A strong foundation in anatomy, physiology, and medical terminology is essential to deliver high-quality patient care in diverse clinical environments.
*Duties*
* Assess patients’ physical conditions through comprehensive evaluations, including mobility, strength, and functional ability.
* Develop and implement individualized *care plans* tailored to patient needs across *orthopedics*, *geriatrics*, *pediatrics*, and other specialties.
* Administer therapeutic interventions such as *trigger point therapy*, manual therapy, and exercise programs to improve patient outcomes.
* Coordinate *discharge planning* and facilitate smooth transitions from hospital or clinic to home or community settings.
* Monitor patient progress regularly and adjust treatment strategies accordingly.
* Document treatment sessions accurately using *EMR systems* and maintain detailed patient records.
* Educate patients and their families on exercises, injury prevention, and health maintenance techniques.
* Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
* Participate in ongoing professional development activities to stay current with advancements in *physiotherapy* practices.
*Skills*
* Extensive knowledge of *anatomy*, *physiology*, and *medical terminology* relevant to physical therapy.
* Proven experience in various clinical settings such as *acute care*, *hospital*, *home & community care*, *outpatient clinics*, and *home health* services.
* Skilled in creating effective *care plans* for diverse populations including *geriatrics*, *pediatrics*, and orthopedic patients.
* Proficiency with *EMR systems* for documentation and patient management.
* Strong understanding of rehabilitation techniques such as *trigger point therapy* and manual therapy methods.
* Ability to perform comprehensive assessments for both inpatient and outpatient cases.
* Excellent communication skills for educating patients and working within multidisciplinary teams.

Pay: $51,000.00 - $68,000.00 per year
