Salary: $253,049/Annually - Salary is Negotiable (Employees will receive 5% below the negotiated pay rate during their probationary status.)

Responsible To: Medical Director

Summary: Provides and manages direct medical evaluation, diagnosis, procedures, and care for patients within a recognized area of subspecialty practice. Provide consultation and advice to others as a highly specialized practitioner. Continuation of this position is contingent upon funding allocations.

Minimum Qualifications: Applicants please clearly state how you meet these qualifications on the application/resume.

• Medical Doctor’s degree: Wisconsin’s Physician License.
• State of Wisconsin and Federal DEA Certification.
• Medical Specialty License or Certification.
• Board certified or Board Eligible in Specified Area of Medical Specialty.
• Must be credentialed.

• Must provide a copy of diploma/degree, license, or certification upon employment.
• Background disclosure form. (BID)
• Employees are required to have proof of seasonal influenza vaccination; medical or religious exemption may be requested upon hire.
• Three (3) Current letters of references (personal, professional, and past employment).
