Job Description
CareConnectMD (CCMD) is a value-based care organization and ACO that partners with physicians to deliver high-quality, cost-effective care to medically complex, high-needs, and post-acute patient populations. CCMD provides financial incentives, operational support, on-the-ground care coordination, and data-driven insights so providers can improve outcomes while increasing shared savings. Built specifically for skilled nursing, long-term care, and frail elderly populations, CCMD helps physicians succeed clinically, financially, and operationally with minimal disruption and full flexibility.
Physician – Post-Acute / Value-Based Care
Salary: $240,000–$270,000 annually- pending experience
Productivity bonus potential (subject to change – at the company’s discretion)
- Monthly Auto Allowance: $600
- CME: $3,500 annual allowance plus 3 CME days
- Time Off: PTO (6.46 hours per pay period) and Sick Leave (48 hours frontloaded) provided
- Malpractice Insurance: Covered by the organization

What You’ll Do
- Provide physician coverage across 8–20 skilled nursing and long-term care facilities
- Create your rounding schedule for skilled, custodial, and ACO REACH patients
- Perform and document timely visits per regulatory and clinical guidelines
- Manage skilled patients (min. 2 visits/week) and custodial patients (monthly visits)
- Participate in daily rounds for ACO REACH patients with medical director
- Coordinate with NPs, care teams, and call center for communication and follow-up
- Handle appropriate coding, documentation, and discharge summaries
- Participate in QI, Peer Review, and utilization management
- Optional paid weekend/overnight call opportunities
Rounding Model
- Physician leads the rounding strategy
- NP team supports with admissions, routine visits, labs, and family communication
- Urgent pages within 15 minutes; routine within 60 minutes
- Visits prioritized based on acuity, admissions, changes in condition, and family needs
What’s Required
- MD or DO (Internal Medicine, Geriatrics, or Palliative Care preferred)
- Active California medical license, DEA, and BLS
- Strong documentation, communication, and clinical judgment
- Ability to work independently and collaboratively across multiple facilities
- Managed care experience preferred
