*Direct Support Professional*
* Adheres to all written and verbal policies established by Home Care Nursing, Inc., and The Department of Mental Health.
* Follows all written and verbal directives given by the Team Leader or House Lead or House Manager, Program Manager or the Executive Director.
* Fosters a positive work atmosphere by behaving and communicating in a professional manner that promotes cooperation and teamwork with co-workers, supervisors, Consumers, Consumer’s family members, Regional Center Staff, and visitors.
* Develops a working knowledge of all Personal Plans. The Personal Plan is the guiding document for all Consumers in a supported living site. This Plan is evaluated and updated by the planning team at least yearly. The section with goals and action steps is vitally important and drives what we do daily with each Consumer. All staff should have knowledge of and are continually tracking goals with the House Lead.
* Provides support to the Consumer in a manner that considers safety, personal dignity, comfort, and a respectful environment.
* Observes Consumer’s behaviors, legibly documents concise and accurate observations in charts and progress notes, and immediately reports any unusual or dangerous behaviors to the immediate supervisor. The progress notes are derived from the Personal Plan and must be completely and concisely filled out, signed, and dated by Staff on each shift.
* Assists the Consumer with daily living, encouraging independence in behavior and decision making, to include healthy meals, assisted home chores, assisted community activities assisted shopping, assisted personal care. Not limited to any of the above and most of all needs of the Consumer, medical, personal, socially, personal calls, family members, friends, etc.
* Dispenses all medication in accordance with doctor’s orders and properly documents actions taken on the MAR. The Direct Support Staff must be aware of and sign that they have reviewed and will monitor for side effects within the MAR.
* Remains with Consumers while on shift, actively interacts with Consumers during all waking shift hours, and remains on shift until properly relieved by other trained staff.
* Prepares injury and incident reports and Doctor’s consultation reports per supervisor’s instructions.
* Participates in Site and Regional Center training and staff development programs to include mandatory training and other necessary training as directed.
* Participates as an integral part of the planning team for the Consumers. This includes the attendance of staff meetings (whenever applicable), meetings with Regional Center Staff in the planning of the Personal Plan goals for the Consumer, meetings regarding behavior support, and any other Consumer specific training or meetings.
* Participates with Consumers in all activities they are involved in and demonstrates a positive and cooperative attitude, providing a positive role model for Consumers as well as other staff.
* Performs other duties, as assigned by supervisors.
