Overview:
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will have experience in healthcare, with a focus on utilization management, resident assessments and care planning in accordance with state and federal regulations. This role is responsible for ensuring the accuracy and completeness of medical documentation and coordinating patient care within our organization. Must have a Registered Nurse License in the State of Michigan.
Duties:
- Implementation and ongoing evaluation of each residents comprehensive care plan.
- Conduct thorough case management assessments to identify potential issues and develop effective discharge plans.
- Accurate completion of all MDS assessments and any supporting assessments or clinical documentation.
- Provide critical care support as needed to ensure optimal patient outcomes.
- Develop and implement effective utilization management strategies to optimize patient care and reduce unnecessary costs.
- Interpreting rules, regulations and coverage guidelines and acting as primary resource for problem solving in regards to the SNF prospective payment system and quality reporting program.
Skills:
- Strong background in healthcare, with a focus on utilization management, discharge planning, and medical documentation.
- Proficiency in medical terminology and the ability to use it accurately in written and verbal communication.
- Experience working as a nurse or in a related healthcare role.
- Knowledge of health information management principles and practices.
- Experience with electronic medical records and other healthcare software systems.
- Excellent critical thinking and problem-solving skills.
- Ability to work effectively in a fast-paced environment with multiple priorities.
- Strong organizational and communication skills, with the ability to work collaboratively with healthcare professionals and patients.
