Position Overview:
* Provide physical therapy evaluations and treatments for residents in skilled nursing facilities
* Develop and implement individualized care plans
* Collaborate with interdisciplinary teams (nursing, OT, ST, physicians)
* Complete timely and accurate documentation
* Educate residents and families on therapy plans and progress
Requirements:
* Active Georgia Physical Therapist license
* Minimum 1 year experience in a nursing home or SNF setting
* Strong knowledge of Medicare/Medicaid guidelines and skilled documentation
* Excellent communication and time-management skills
* Ability to work independently and as part of a team
