Our main location will be located at Brookdale Baywood ALF but our position will require our PT to work at our SNF location in Fountain Hills as needed.
The Physical Therapist is responsible for providing rehabilitative therapy to patients upon completion of the evaluation and plan of care following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.
This position reports to the Director of Rehabilitation Services.

*MINIMUM QUALIFICATIONS*
* Must possess a current/ active Arizona License.
* Knowledge of the principles of PT Scope of Practice.
* Demonstrated ability to communicate proficiently both orally and in writing in the English language.
