The Opportunity
The Case Manager participates as a member of the Foundry Langley team. The Case Manager helps youth and young adults with complex needs by coordinating care, preforming follow ups, and providing service navigation and resource information. The Case Manager will be available on the floor during drop in times supporting youth and families with service navigation, orientation to the centre and supporting the lounge where needed. The Case Manager will oversee and provide waitlist management and follow up support for specialized services at the centre such as Psychiatry consults and Gender Affirming Care. The Case Manager will work collaboratively with the Clinical Coordinator, to facilitate monthly team based care meetings and will provide other specialized supports where needed.

POSITION: Permanent, Full-Time
HOURS OF WORK: 37.5 hours per week
WORK SCHEDULE: Monday to Wednesday: 11:30am-7:30pm
Thursday, Friday: 9-5pm.
SALARY RANGE: CSSEA Wage Grid 12: $ $33.49hourly (depending on experience and qualification)
BENEFITS: Extended health, dental, vision care, Group TFSA/RSP
VACATION: As per ESSS policy

Some Key Duties and Responsibilities:
• Engage and build positive relationships with youth and families accessing Foundry Langley during drop in who may not be engaged with community supports.
• Support the orientation and intake of new participants during drop-in and support youth accessing the lounge services.
• Coordinate care among internal and external community partners for youth and families.
• Engage and provide resource navigation to youth and families to support their goals.
• Oversee waitlist management and provide follow-up to specialized services such as psychiatry consults and gender affirming care.
• Document, implement, and provide input into the evaluation of the program.
• Gather information relevant to the participant's strengths, needs and risks by interviewing, observing behavior, meeting with caregivers and service providers, and using a variety of inventories, checklists and questionnaires. Review the information gathered to identify strengths, needs and risks.
• Plan, prepare, and conduct group or individual sessions using techniques such as active listening, conflict resolution, and basic psycho-educational to resolve the identified needs and risks.
• Provide skill building in areas such as life skills, anger management, conflict resolution, mediation or other self-management techniques.
• Organize resources to address community gaps and plan youth wellness activities within the center and community to meet the needs of youth.
• Collect and maintain records and statistics relevant to the position, reporting to the Clinical Coordinator.
• Develop and maintain positive working relationships with all Encompass and Foundry Langley staff, volunteers, partners, and community.
• Attend and provide activities at community events on behalf of Encompass.
• Attend supervision, team-based and partnership meetings as required.
• All other duties as they relate to the Foundry Langley which may be assigned from time to time.
Qualifications:
1. Education and Knowledge
• Completion of Bachelors Degree in human/social services or related field
• Completion of Bachelor of Social Work preferred
2. Training and Experience
• 1 year recent related experience or an equivalent combination of education, training and experience.
• Direct service delivery experience in community social services sector, and demonstrated knowledge of and connection with existing social service supports, educational, skill building and employment services in the community is an asset.
• Current First Aid Certification required.
3. Skills and Abilities
• Excellent interpersonal and conflict resolution skills to interact effectively and build relationships with program staff, families, funders, volunteers, community, agencies and other organizations.
• Ability to operate within professional boundaries when developing and maintaining relationships with participants and stakeholders.
• Solution-oriented to meet challenges facing participants served through Foundry Langley, including but not limited to youth homeless, substance use, and mental health concerns.
• Knowledge of building resilience and community belonging through the youth drop-in model.
• Demonstrated experience in competently assessing the needs of youth to support them in achieving their goals
• Demonstrated ability to write reports, maintain up to date and relevant case notes and compile statistic to support success measure and program development.
• Ability to work in a fast paced environment and respond effectively to crisis situations
• Strong interpersonal skills, with demonstrated ability to work, independently, within a team setting and as a part of a multi-disciplinary team.
• Ability to establish and maintain effective working relationship with those served
• Preference will be given to individuals with crisis intervention, suicide prevention, conflict resolution, and bullying and or gang awareness.
• Experience working with vulnerable youth 12-24 years of age.
• Experience or knowledge of issues such as substance abuse, mental health issues, homelessness and exploitation.
• Demonstrated knowledge and application of trauma-informed and culturally safe principles of care.
• Demonstrated knowledge of relevant Acts, pertinent regulations, policy, procedures and other statutory and certification requirements (such as the Mental Health Act; Adult Guardianship Act; Child, Family and Community Service Act; and Freedom of Information and Protection of Privacy Act).
• Knowledge of systemic issues and risk factors facing underrepresented groups including 2SLGBTQIA+ and Indigenous youth and young adults.
4. Other Information
• Able to work within a flexible work schedule including evenings and possibly weekends.
• Must possess a valid BC Class 5 Drivers' license, have a reliable vehicle, business insurance and drivers abstract review (mileage paid for work use)
• Provide primary source credential verification
• Must have a working knowledge of computers and Microsoft Office software
• Police information check, vulnerable sector
