We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, develop, and implement personalized treatment plans to help patients regain movement, reduce pain, and improve their overall quality of life. Whether working in inpatient hospital settings, outpatient clinics, or home health environments, you will play a key role in guiding patients through their recovery journeys with compassion and expertise. Your expertise in physiology, anatomy, and physiotherapy techniques will ensure that every patient receives comprehensive and effective care tailored to their unique needs.
*Responsibilities*
* Conduct thorough patient assessments to determine physical capabilities and limitations using medical terminology and anatomy knowledge.
* Develop individualized care plans that incorporate evidence-based physiotherapy techniques, including trigger point therapy and orthopedics.
* Implement treatment interventions for diverse populations, including pediatrics, geriatrics, and those recovering from surgery or injury.
* Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless patient transitions across inpatient, outpatient, home health, and community care settings.
* Monitor patient progress regularly, adjusting care plans as needed to optimize recovery outcomes.
* Educate patients and their families on physiotherapy exercises, injury prevention strategies, and home care routines.
* Maintain accurate documentation within electronic medical record (EMR) systems and ensure compliance with healthcare regulations.
*Requirements*
* Proven experience in inpatient hospital settings, outpatient clinics, or home health environments with a strong understanding of acute care protocols.
* Demonstrated expertise in physiotherapy techniques such as trigger point therapy and orthopedics.
* Knowledge of physiology, anatomy, medical terminology, and care planning processes.
* Prior experience working with diverse patient populations including pediatrics and geriatrics.
* Familiarity with EMR systems for documentation and patient record management.
* Strong communication skills to effectively collaborate with healthcare teams and educate patients.
* Ability to perform discharge planning efficiently while ensuring comprehensive patient care continuity.

Pay: $45.67 - $65.00 per hour
