Contract Details

• Location: Goshen, Indiana
• Setting: Long-Term Care (LTC)
• Contract Length: 4–6 Weeks (Block Booking)
• Guaranteed Hours: 40 Hours per Week
• Shift Options:
  • 2:00 PM – 10:00 PM
  • 10:00 PM – 6:00 AM
• 8-Hour Shifts
• Every Other Weekend Required
• Structured Rotating Schedule
• Pay Rate: $33.00 per hour

Schedule

Week 1: Monday – Saturday
Week 2: Sunday + Tuesday – Friday

• Rotating 2-week schedule

Requirements

• Active and unrestricted Indiana LPN license
• Minimum 1 year of Long-Term Care experience required
• Current CPR certification
• Strong medication administration skills
• Ability to work efficiently in a fast-paced LTC environment
• Professional documentation and communication skills

Responsibilities

• Provide skilled nursing care in a long-term care setting
• Administer medications and treatments as ordered
• Monitor residents and report changes in condition
• Maintain accurate and timely documentation
• Collaborate with interdisciplinary care team
• Support a safe and structured resident care environment
