What You Will Do
Deliver patient-centred care through individualized assessment, treatment planning, and ongoing evaluation of progress.
Coordinate rehabilitation services, including regular RC sessions, scheduling, and treatment plan updates.
Liaise with clients, families, referral sources, funders, and internal teams to support continuity of care.
Contribute to team excellence through collaboration, sound judgement, integrity, and continuous professional development.
What You Will Need To Be Successful
You have a Bachelor of Social Work or a Master of Social Work.
You are a registered member in good standing with the Alberta College of Social Workers.
1 to 2 years of community experience is an asset.
You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions.
You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently.
You have ability to travel within the Hawkesbury community as part of the CBI Health Home Health Services team.
