We are seeking a proactive, people-first Patient Services Coordinator who takes initiative, anticipates needs, and leads intake and administrative processes with confidence and compassion. This is a highly visible, patient-facing role within our Transitional Care Unit (TCU) and is critical to ensuring smooth admissions, coordinated discharges, and strong day-to-day operations.
This role goes beyond task completion — we are looking for someone who steps up, solves problems, and helps keep the unit running smoothly, supporting both patients and clinical teams.
This position is central to patient flow, family experience, and operational success. The right candidate will not wait to be told what to do — they will lead from the front, anticipate challenges, and help ensure patients receive respectful, coordinated care from the moment they arrive.
Shift Details
Location: Main Office (In-person role; travel to Kitchener TCU required) Pay Range: $22.00 – $25.00 per hour Shift Timing: Between 7am to 7pm (7.5 hours) Schedule: Every other weekend and rotational coverage required Key Responsibilities
Actively collaborate with clinical and operational teams to ensure a seamless, patient-centered admission experience. Coordinate and facilitate patient intake and discharge processes in a timely, professional, and compassionate manner. Act as a liaison between families, internal teams, external partners, and government agencies, to coordinate bed offers and service transitions. Respond to inquiries and provide clear information regarding services and care processes. Collaborate with the clinical team to ensure a seamless and empathetic admission experience. Support operations by coordinating schedules and assisting with shift coverage as required. Maintain accurate and up-to-date administrative records and KPI's including documentation required for reporting, analytics, and billing purposes. Develop quarterly reporting packages for regulatory compliance. Complete billing-related administrative tasks and ensure documentation meets organizational and regulatory standards. Support overall TCU administration by monitoring adherence to organizational policies, procedures, and operational guidelines. Identify concerns, risks, or gaps and escalate issues appropriately to clinical leadership or management. Make recommendations for process improvements. Ensure all administrative practices comply with privacy, confidentiality, and legislative requirements. Requirements

Sound critical thinking and problem-solving abilities. Confidence working independently while collaborating effectively with multidisciplinary healthcare teams. Effective interpersonal skills and the ability to collaborate with multidisciplinary teams. Comfort working with administrative systems and standard office technology. Willingness and ability to travel between the Kitchener TCU and the main office. Availability to work every other weekend and rotational shifts. Valid driver's license with a clean driving record.
