Case Manager – East Case Management (Full-Time Permanent)

The Role and Opportunity

The Canadian Mental Health Association (CMHA) – Toronto is looking for a Case Manager for our East Case Management Program. This Case Management program offers intensive case management services as part of the continuum of mental health services and supports for people with mental illness. Intensive case management promotes independence and quality of life through the coordination of appropriate services and the provision of constant and on-going support as needed by the client. The direct involvement of the client and the development of a caring, supportive relationship between the case manager and the client are integral components of the service. Service is provided in the community.

Salary: $54,943 – $60,203

Hours of Work: 35 per week

Responsibilities

Reporting to the Program Manager, the Case Manager will work with the interdisciplinary team to support their clients in the community. They will be required to participate in person team meetings and provide regular updates to managers.  

The aim of the Case Manager will be to assist in the development of a service care plan, access appropriate services and provide case management for clients accepted to the case management service. This will begin with intake of client information to assess their needs and identify supports required; this will be done through effective consultation and collaboration with the client. 

This role will also involve direct service provision/intervention with a strength-based trauma informed approach. The Case Manager will also develop, implement, and monitor the service plan and conduct follow-ups with the clients as needed. The Case Manager will advocate on behalf of the client and connect them with community resources as well as establishing effective working relationships with staff of other agencies. 

Experience & Education Required

The ideal candidate will have:

  • An undergraduate degree or diploma in social services or a related field.
  • A minimum of three years' experience in direct service delivery to adults with serious psychiatric disabilities (experience in community mental health is an asset).
  • Strong knowledge of crisis intervention/prevention, addictions, recovery-oriented practice, culturally competent and trauma-informed care, and severe/persistent mental illness.
  • A solution-focused approach, strong crisis management skills, and experience working with diverse communities from an anti-racism, anti-oppression, and non-homophobic lens.
  • The ability to work both independently and within a multidisciplinary team, with effective analytical, assessment, and decision-making skills.
  • A valid Ontario Driver's License, access to a vehicle, and appropriate insurance coverage.
