Position Summary:
Wasauksing First Nation is accepting applications for a Nurse Practitioner. Under administrative direction and within the scope of practice a Nurse Practitioner, they are responsible for the provision of primary healthcare and supervision of the nursing staff within the clinic. The Nurse Practitioner assesses and diagnoses client health for acute illnesses; supports clients to manage chronic illnesses; ensures referral if needed for all unattached clients. The Nurse Practitioner works with the client; other practitioners and the community to promote healthy lifestyles and disease prevention. The Nurse Practitioner collaborates and consults with members of an interdisciplinary team to address issues experienced by clients and within the community, and situations that are outside their scope of practice or exceed their knowledge and/or skill. The Nurse Practitioner's practice is guided by the College of Nurses, Standards of Nursing Practice (NP), and the Registered Health Professions Act.
Job Duties & Responsibilities:
1. Assess and diagnose client health
- Provide ongoing primary health care to clients, including assessment, diagnosis, treatment, referral for specialists or out-of-scope circumstances, and complete minor procedures
- Order and interpret investigative screenings and tests to aid in diagnosis and treatment
- Provide counselling, education and follow-up with clients to treat and manage illnesses
- Determine the presence of existing and potential health problems with a focus on lifestyle, psychological, socio-cultural and environmental risk factors that may influence the client's health status
- Complete client medical record within the electronic medical records database in an accurate and timely manner
- Collaborate and consult with physicians to address circumstances outside of their scope of practice or exceeding their knowledge and skill
- Collaborates with providers (both in-house and external) and clients to ensure management, referrals and information sharing to maintain the client's continuity of care
2. Support the management of chronic illness in clients
- Monitor the ongoing therapy of clients with chronic, stable illness by providing pharmacological, complementary and counselling interventions
- Prescribe therapies to manage the chronic illness
3. Work with clients and the community to promote healthy lifestyles, disease prevention and community health
- Support clients and their families to take increased responsibility for their health by involving them in risk factor and health problem identification, goal setting and the choice of interventions for disease treatment, prevention and health promotion
- Implement strategies to promote health and prevent disease with individuals, families and groups
- Synthesize information from individual clients to identify broader implications for health within the family and the community
4. Performs administrative duties in support of the organization's goals and objectives
- Participate in and promote the interdisciplinary provider model
- Provide consultation services to community groups, agencies and IIPCT staff
- Assist in the development and evaluation of policies, protocols and procedures to improve services to clients and to promote cooperative and efficient staff communications
- Assist in the development, planning and evaluation of treatment, education, counselling and health promotion programs
- Prepare statistical reports for reporting and evaluation purposes
5. Perform other duties as assigned within the scope of the position
- Engage in professional development to maintain current skills and knowledge to work to the scope of the position
- Assist in the development and implementation of the organization's quality improvement initiatives
- Network with partner agencies to improve practices, enhance services and move forward shared initiatives relevant to the position
6. Perform duties as a Clinical Manager and leader in primary care to assigned nursing staff.
- Oversee the operations of nursing within the assigned clinical team
- Complete performance evaluation on all assigned registered staff (does not necessarily include-contracted nursing staff for specific assignments)
- Review with the Director of Health-any and all contractual agreements regarding EMR (Telus); Referrals Apps (OCEAN); concerns and issues-impacting care (consultation with DOH)
- Ensure prompt and accurate reports on clinic staff through HR Bamboo
- Provide assistance and consult with DOH on questions/concerns regarding quarterly/final reports to the Ministry of Health-Primary Care Division.
Secondary Duties
- Perform other duties as necessary.
- Adhere to all policies and procedures of Wasauksing First Nation and any other relevant policies and procedures from external agencies and partners
- Perform other duties within the scope of the position as assigned by the Manager or Supervisor
Education & Experience:
- Minimum Bachelor's Degree of Science in Nursing from an accredited program in Ontario
- Nurse Practitioner Certificate from an accredited program in Ontario
- Current registration in good standing in the Extended Class with the College of Nurses of Ontario
- Current registration in good standing with the Nurse Practitioner Association of Ontario
- Three years to five years' experience working as a Nurse Practitioner
- Current CPR Level C and First Aid Certificate
- Experience in a community health care setting, an asset.
- Experience working in nursing within First Nation populations, an asset.
- Experience working within an electronic medical record system, an asset.
Other Requirements:
- Ability to speak Ojibway would be an asset
- Evidence of knowledge of Wasauksing First Nation's history and culture would be an asset
- Knowledge of First Nation health issues
- Valid Driver's License and reliable personal vehicle
- Willing to work flexible hours as required, including evenings and weekends
- This position requires the individual to work with various families, youth and children. To protect children, youth and vulnerable persons, Wasauksing First Nation requires the individual to provide a vulnerable sector. Or criminal record check
