*Responsibilities*
* Evaluate patients' physical and mental health to develop personalized treatment plans.
* Assist patients with activities of daily living (ADLs) to enhance their independence.
* Collaborate with interdisciplinary teams for discharge planning and ongoing care.
* Implement therapeutic interventions tailored to individuals with disabilities.
* Document patient progress and adjust care plans as necessary to ensure optimal outcomes.
* Provide education and support to patients and families regarding treatment options and strategies.
Job Type: Full-time
Pay: Up to $55.00 per hour
