We are seeking a dedicated and skilled *Physical Therapist* to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including inpatient, outpatient, hospital, home health, and community care environments. This role requires a strong foundation in physiotherapy principles, anatomy, physiology, and medical terminology to develop and implement effective care plans tailored to individual patient needs. The Physical Therapist will play a vital role in improving mobility, reducing pain, and enhancing the quality of life for diverse patient populations, including pediatrics and geriatrics.
*Duties*
* Assess patients’ physical conditions through detailed evaluations of mobility, strength, and functional abilities.
* Develop and implement personalized *care plans* based on patient assessments and treatment goals.
* Administer therapeutic interventions such as *trigger point therapy*, manual therapy, exercise programs, and modalities to promote healing and recovery.
* Manage *discharge planning* to ensure seamless transition from hospital or clinical settings to home or community environments.
* Provide specialized care for *orthopedics*, *geriatrics*, *pediatrics*, and patients with acute or chronic conditions.
* Collaborate with multidisciplinary teams to coordinate patient care in *inpatient*, *outpatient*, *hospital*, or *home & community care* settings.
* Document patient progress and treatment outcomes accurately using *EMR systems*.
* Educate patients and caregivers on injury prevention, rehabilitation exercises, and health maintenance strategies.
* Stay current with advancements in *physiotherapy*, including new techniques such as trigger point therapy and physiologic approaches.
*Skills*
* Strong knowledge of *anatomy*, *physiology*, and *medical terminology* relevant to physical therapy practice.
* Proven experience in various clinical environments such as *hospital*, *home health*, outpatient clinics, or inpatient facilities.
* Expertise in developing effective *care plans* for diverse populations including pediatrics and geriatrics.
* Familiarity with *discharge planning* processes to facilitate patient independence post-treatment.
* Ability to perform comprehensive assessments for patients with orthopedic or neurological conditions.
* Excellent communication skills for patient education and interdisciplinary collaboration.
* Experience with *EMR systems* for documentation and record keeping.
* Knowledge of treatment techniques such as manual therapy, trigger point therapy, and therapeutic exercises.

Pay: $85,000.00 - $105,000.00 per year
