Primary Care Physician Role Overview:

The Primary Care Physician provides comprehensive primary care services to a panel of veterans, ensuring continuity of care within a multidisciplinary healthcare team.

Responsibilities Include:

• Veteran-Centered Care:  Deliver comprehensive services emphasizing health maintenance, disease prevention, patient education, and chronic disease management.
• Patient Aligned Care Team (PACT) Model:  Implement collaboration and coordination among multidisciplinary healthcare team members.
• Quality Improvement:  Participate in projects to enhance delivery of care.
• Clinical Leadership:  Serve as a mentor and engage in educational activities.
• Healthcare Coordination:  Coordinate care involving specialty and ancillary services.
• Use of Technology:  Utilize the VA's electronic medical record system; embrace telehealth.
