Position Summary
The HMIS/Coordinated Entry Coordinator is responsible for the daily administration and oversight of the Coordinated Entry System (CES) and its integration with the Homeless Management Information System (HMIS) for New Life Discovery Project. This role ensures that individuals and families experiencing homelessness are prioritized for housing and services based on vulnerability and need, rather than a first-come, first-served basis. The Coordinator acts as the primary liaison between New Life Discovery Project and the CoC, HMIS and state/federal regulatory bodies.
Key Responsibilities
1. System Administration & Compliance
* Workflow Design: Develop and maintain standardized CE workflows within the HMIS to track referrals, assessments, and housing placements.
* Policy Oversight: Ensure the CES complies with HUD's Coordinated Entry Management and Data Guide and California's specific data sharing and privacy laws ( e.g., CalECP A).
* Data Quality: Monitor HMIS data integrity, ensuring all Clients of NLDP are recording Coordinated Entry "Events" and "Assessments" (like the VI-SPDAT or local equivalent) accurately and timely.
2. Coordination & Case Conferencing
* By-Name List (BNL) Management: Maintain an up-to-date, real-time By-Name List of all individuals experiencing homelessness in HMIS system Wellsky.
* Lead Case Conferencing: Facilitate weekly or bi-monthly meetings with housing navigators and to troubleshoot barriers for the most vulnerable clients.
* Referral Management: Manage the "Community Queue," matching eligible households to available permanent supportive housing (PSH), rapid re-housing (RRH), and other subsidies.
3. Training & Technical Assistance
* Provider Training: Design and deliver training for Access Point staff on assessment tools, trauma-informed intake, and HMIS data entry.
* Technical Support: Serve as the first point of contact for NLDP experiencing technical issues with the CE module in HMIS.
4. Reporting & Analysis
* Performance Metrics: Generate monthly and quarterly reports on system performance (e.g., length of time homeless, successful placements, and racial equity impacts).
* State Reporting: Assist in preparing data for California-specific mandates, such as the Homeless Data Integration System (HDIS) and HHAP grant reporting.
Qualifications:
* Education: Bachelor's degree in Social Work, Public Policy, Data Science, or a related field (Master's preferred).
* Experience: 2-4 years of experience in homeless services, HMIS administration
* (Bitfocus Clarity, WellSky, etc.), or social service program coordination.
* Technical Skills: \* Advanced proficiency in HMIS software.
* Strong data analysis skills (Excel is required; SQL or Tableau is a plus).
* Knowledge Base: Deep understanding of Housing First, Trauma-Informed Care, and HUD CoC/ESG regulations.
Required Competencies:
* Racial Equity Lens: Ability to analyze data to identify and address disparities in housing access and outcomes.
* Communication: Exceptional ability to translate "data-speak" into actionable insights for frontline case managers and stakeholders.
* Adaptability: Comfortable navigating the evolving landscape of California's homelessness response system.

Pay: $65,000.00 - $90,000.00 per year
