At *Meadow Creek Post-Acute *we offer the highest quality care to all of our residents. We provide skilled nursing care, customized therapy services and licensed professional assistance. Meadow Creek is located in Paramount, CA and is a 104-bed Skilled Nursing Facility specializing in long-term care, short-term rehabilitation and in-house dialysis within our sub-acute unit.
*Meadow Creek Post Acute *is seeking a highly skilled and compassionate *Speech Language Therapist* to join our team. As a *Speech Therapist,* you will work with patients of all ages who have difficulties with speech, language, communication, and swallowing. Your expertise will play a crucial role in helping patients overcome their challenges and achieve better communication abilities.
* Facility Type: Subacute, Skilled Nursing (Long/Short Term), 140 bed
* Location: 7039 Alondra Blvd., Paramount, CA 90723
* Schedule: PRN
* *Pay Range: *$50-$55 per hour
* Other benefits: Paid orientation and training
*Specific duties include, but are not limited to*:
* Conduct comprehensive assessments to evaluate patients' speech and language abilities.
* Develop and implement individualized treatment plans, targeting specific speech, language, and swallowing goals.
* Provide therapeutic interventions, including but not limited to, articulation therapy, language development therapy, voice therapy, and cognitive training.
* Monitor and document patients' progress and make adjustments to treatment plans, as necessary.
* Collaborate with other healthcare professionals, such as occupational therapists, physical therapists, and nurses, to coordinate patient care.
* Educate patients, families, and caregivers about speech and language disorders, treatment strategies, and home exercises.
* Participate in team meetings and case conferences to discuss patients' progress and make recommendations for discharge planning.
*REQUIREMENTS:*
* Valid state licensure as a Speech Language Therapist
* Active CPR/BLS Certification
* Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
* Is in good standing with all regulatory agencies and licensing boards is required
* Interest in working with the geriatric population
* Master's degree in Speech-Language Pathology from an accredited program.
* Strong knowledge of speech and language disorders, evaluation techniques, and evidence-based therapeutic interventions.
* Excellent communication and interpersonal skills.
* Ability to work effectively in a multidisciplinary team
* Proficiency in computer skills and electronic medical records systems.
* Strong organizational and time management skills.
* Compassionate and patient-centered approach to care.
* Ability to adapt to changing priorities and thrive in a fast-paced environment.
*EXPERIENCE:*
* With Skilled Nursing or Long-Term Care experience is preferred
* Experienced Speech Language Pathologist (SLP) or Speech Therapist (ST)
