We are looking for a Full-Time RN, Case Manager to join our Hospice team. The service area will be Clarksville, Mecklenburg and Halifax county.

ROLE SUMMARY

The Hospice Case Manager identifies and coordinates care to support terminally ill patients and their families at home, in skilled nursing facilities or at residential care facilities. On average, the Full-Time Hospice Case Manager's case load will be 12-15 patients. The Hospice Case Manager will determine the frequency of patient/family contacts based on the assessment of need, with a minimum of one visit per week. The Case Manager works closely with the Medical Director or Primary Care physician, while utilizing teaching, assessment, and intervention skills to provide comfort care to maximize the quality of life for the patients and their families.

RESPONSIBILITIES

  1. Assesses home care needs, being aware of the physical, emotional, and spiritual aspects of care; gathers data on social, economic and cultural factors which may influence health, well-being and quality of life.

  2. Provides direct care as prescribed in the Interdisciplinary Plan of Care in order to maintain the highest level of comfort and quality of life and assuming primary responsibility for case management.

  3. Authorizes, coordinates and supervises care, as prescribed in the Interdisciplinary Plan of Care, with contracted vendors in order to meet the needs of the patient.

  4. Evaluates and performs ongoing assessments; revises initial written Plan of Care in collaboration with the interdisciplinary team weekly or as the needs and conditions of the patient/family change.

  5. Supervises and maintains effective communication with other hospice personnel involved with patient care. This may involve formal and informal team meetings in addition to IDT (Inter-Disciplinary Team Meeting).

  6. Documents accurate and ongoing assessment of patient status via a variety of mediums of communication (verbal, written, email, computer documents). Documents patient care reflecting nursing interventions, patient response to care, patient needs, problems, capabilities, limitations, and progress toward goals. Documentation includes evidence of appropriate patient/family teaching; the understanding of these instructions is noted in the medical record.

  7. Communicates regularly with the patient's physician, most often the Medical Director, regarding changes in the patient's Plan of Care.

  8. Establishes the Home Health Aide's Plan of Care; indirectly supervises the HH Aide by asking scripted questions of the family regarding the care received and directly supervised during joint visits.

QUALIFICATIONS AND REQUIREMENTS Education: All applicants must possess an RN license to work in the state of Virginia and be CPR certified. Must have a valid drivers license and provide own transportation.

Experience: Previous hospice nursing experience preferred.

Pay: $74, $80,000.00 per year
