Nurse Practitioner

Position Title: Family Nurse Practitioner – Nephrology

Position Overview:
We are seeking a skilled and compassionate Family Nurse Practitioner to join our Nephrology Department. In this role, you will provide comprehensive care to patients with kidney-related conditions, working closely with our team of experienced nephrologists to deliver high-quality, patient-centered care.

Key Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans
• Manage chronic kidney disease (CKD), hypertension, and other nephrology-related conditions
• Perform and oversee dialysis-related procedures, ensuring quality and continuity of care
• Educate patients and families on kidney health, treatment options, and lifestyle modifications
• Collaborate with multidisciplinary healthcare teams to optimize patient outcomes
• Participate in research initiatives and quality improvement projects within the department

Qualifications:

• Master's degree in Nursing from an accredited Nurse Practitioner program
• Current Nurse Practitioner licensure in (State)
• Nephrology certification (CNN-NP or equivalent) preferred
• Minimum of (number) years of clinical experience in nephrology or a related field
• Proficiency in dialysis-related procedures and interpretation of laboratory results
• Excellent communication and interpersonal skills
• Demonstrated commitment to patient-centered, evidence-based care
