ConnextCare is seeking an experienced Nurse Practitioner to add to its growing Oswego office.
Principal Accountabilities:
Provide direct patient care as outlined in the employment contract, including diagnosis, treatment and referral
Perform services within the scope of practice. Perform services under the guidance of a collaborating physician if indicated
Report any clinical deficiencies to the chief medical officer
Maintain a proper and accurate medical record, complying with all local, state and federal requirements
Adhere to all medical staff by-laws, administrative and personnel policies and procedures
Participate with in-service workshops, continuing education, quality improvement and peer review
Complete encounter forms in a timely manner, indicating services rendered and to code appropriately
Participate with the quality assurance team in development of goals and objectives for the Health Care Plan, and various audits to monitor that these goals and objectives are being carried out
Participate in rotation of evening, weekend and on call schedules
Interpret the role of the Nurse Practitioner to the community
Work closely with the Chief Medical Officer and medical staff in improving efficiency and productivity
Maintain membership in professional organization(s) to develop knowledge and resources through networking, continuing education, and participation in national, regional, and/or local activities
Other duties and responsibilities as may be requested by the Chief Medical Officer and/or Administration
Assist with and promote all processes associated with the Coleman initiative
Promoting Patient Centered Medical Home initiatives
Requirements
Knowledge/ Education Requirements:
A. Valid/active NYS RN and NP licensure
B. Masters degree in a Nursing
C. Experience in family practice preferred, but not required
D. Computer proficiency in electronic health records, health information technology and health informatics required
E. Promote the organization's code of conduct, safety rules, and adherence to all company policies
F. Carry out job responsibilities in an ethical, effective, comprehensive and professional manner
G. Cooperate, work, and communicate with staff, supervisors, management, and patients in a professional manner
H. Display compassion and work well in emergent and stressful situations
I. Demonstrate and enforce standards for safe, quality patient care, setting high standards by example
J. Must be keenly aware of the importance of confidentiality in all aspects of the position
Salary Description
100,000-120,000 DOE
