Hospice Weekend LPN

• Provide on‑call coverage during scheduled weekend hours, responding promptly to patient and family needs
• Perform in‑home visits for urgent assessments, symptom management, and changes in condition
• Implement nursing interventions based on the established plan of care and hospice protocols
• Communicate changes in patient status to the RN Case Manager, interdisciplinary team, and on‑call leadership
• Document all care provided in accordance with state, federal, and agency standards
• Provide emotional support and education to patients and families regarding disease progression, comfort measures, and hospice philosophy
• Collaborate with interdisciplinary team members to ensure continuity and quality of care
• Follow infection control practices and maintain safety standards in all care environments
• Participate in on‑call handoff reports to ensure smooth transitions between shifts
• Maintain accurate medication records and assist with medication reconciliation during visits

• Active Washington State LPN license in good standing
• Minimum 1 year nursing experience; hospice, home health, or acute care preferred
• Valid driver's license and reliable transportation for home visits throughout Snohomish County
• Current BLS certification

Schedule: Saturday & Sunday, 8:00 a.m. – 6:00 p.m. (On‑Call) Location: Snohomish County, WA

$40-45 an hour DOE
