Springpoint at Home is seeking a Full-Time Intake and Scheduling Coordinator to join our team in our Stonebridge At Montgomery location, located in Skillman, NJ

Springpoint at Home provides private duty home care services to our clients.

Responsibilities:

  • Performs initial intake of patients to include general demographic information and patient needs as per agency policies.
  • Coordinates the clinical schedules for all clinical staff to ensure schedules are maintained, updated and communicated to field staff, clients/families, and Home Care Supervisor.
  • Assist with data entry of necessary patient and staff information into the electronic medical record (EMR).
  • Coordinates visits with the clinical staff, and maintains a tracking mechanism to ensure that all visits are verified before being billed or paid out to the caregiver.
  • Processes and coordinates all required clinical documentation and communications.
  • Notifies the RN on call of any client status changes, employee injuries, hospitalizations or staffing issues that impact overall safety or operations management.
  • Performs pre-bill and payroll audit activities. Notifies the Home Care Supervisor of unresolved discrepancies between visits ordered, scheduled, authorized, and documented and follows-up to ensure resolution.
  • Notifies Home Care Supervisor of inability to cover requests for service & call outs in a timely manner.
  • Receives and relays messages for field staff in timely manner.
  • Assist with weekend patient scheduling and on-call coverage.

Education and Experience:

  • High-volume, fast paced scheduling for home health, ensuring compliance with client needs and agency protocols
  • High School Diploma or equivalent.
  • Minimum of 1 year secretarial/administrative assistant experience preferred.
  • Experience in general office equipment, basic business functions.
  • Electronic medical record knowledge preferred
  • Above average word processing/data entry skills/telephone skills
  • Knowledge of payroll process and billing process preferred.
  • Knowledge of medical terminology preferred.

Pay Range: $24-$27/hour

Schedule: 8AM-4:30PM, Monday through Friday - Weekly On-Call Rotation
