About the RoleThe Certified Occupational Therapy Assistant (COTA/L) at Elmwood Healthcare - Home Health (Elmwood Home Care ) will provide direct occupational therapy services to patients in their homes, supporting the rehabilitation process and helping patients achieve their maximum level of independence. The COTA/L will work under the supervision of a licensed Occupational Therapist (OT/R) to implement treatment plans, monitor progress, and adapt interventions based on patient needs.

Key Responsibilities:
Patient Care and Therapy:
Assist in implementing occupational therapy treatment plans designed by the Occupational Therapist (OT) to improve patients' daily living skills, mobility, and overall functional abilities.
- Provide hands-on therapy services, including therapeutic exercises, activities of daily living (ADL) training, and adaptive techniques.
- Educate patients and their families on techniques and exercises to support ongoing therapy goals and enhance self-management.
Assessment and Documentation:
Conduct initial and ongoing assessments of patients' functional abilities and needs, documenting progress and changes in condition.
- Maintain accurate and up-to-date patient records, including therapy notes, progress reports, and treatment outcomes, in compliance with healthcare regulations and company policies.
- Report significant changes in patients' conditions or responses to therapy to the supervising OT.
Care Coordination:
Collaborate with the supervising OT, physicians, and other healthcare professionals to ensure integrated and effective care.
- Participate in care team meetings and contribute to the development and review of patient care plans.
- Communicate effectively with patients, families, and caregivers to provide support, education, and guidance.
Patient and Family Education:
Teach patients and their families about the therapeutic processes, adaptive equipment, and strategies to improve functionality and independence.
- Provide support and encouragement to patients to help them stay motivated and engaged in their therapy.
Administrative Duties:
Ensure all therapy services are provided in accordance with applicable regulations, company policies, and best practices.
- Assist with scheduling therapy sessions and coordinating care activities as needed.
Quality Improvement:
Participate in ongoing training and professional development to stay current with industry standards and advancements in occupational therapy.
- Contribute to quality improvement initiatives and assist in evaluating the effectiveness of therapy interventions.
