Your Role:
- Part-time and PRN positions available, with the opportunity to build a full-time caseload.
- Provide physician-ordered Physical Therapy to patients in their homes.
- Perform initial evaluations, 30-day reassessments, and recertifications/discharges.
- Supervise PTAs (Physical Therapy Assistants).
- Educate patients and caregivers/families on Home Exercise Programs (HEPs).
- Collaborate with other health care professionals within the interdisciplinary team.
- Consult with physicians regarding patient changes and progress.
We Serve:
Our goal is to provide quality care in the comfort of patients' homes, serving homebound patients in Manistee, Mason, Lake, Osceola, and Oceana counties.
We Are Seeking:
- A current Physical Therapy license in the State of Michigan.
- CPR certification and TB test at the time of hire.
- At least 1 year of experience in home health care or a facility (preferred but not required).
- The ability to meet the physical demands of the job.
- Strong communication skills and dedication to quality patient care.
- Reliable transportation.
