Job Overview

The Resident Care Coordinator is responsible for the overall planning, coordination, and implementation of a wide and varied scope of activities and programs designed to meet the unique physical, emotional, and spiritual needs of residents through the community's person-centered, activity-focused programming. Additionally, this position works closely with the community's clinical team to promote open communication regarding the resident's overall well-being.

ROLES AND RESPONSIBILITIES:

· Maintain monthly staffing schedule. Arrange staffing coverage for scheduled absences and provide assistance with finding coverage for unscheduled absences. Maintain updated staff contact list; PRN availability info; records of approved vacation/leave, overtime, and agency use.

· Will assist on floor in the event that staff are missing to reach resident needs and compliance.

· Maintain the Daily Census Report, roster of residents, list of resident family/emergency contacts.

· Maintain an up-to-date roster of Residents.

· Answer telephone; deliver messages to residents and staff, as necessary.

· Complete long term care insurance documentation and submit in a timely manner.

· Keep nursing service staff informed of residents' appointments.

· Prepare daily resident care assignment sheets - ADL assistance, shower and laundry schedules, and other unit activities. Coordinate and track service delivery.

· Maintain required information for monthly billing chart – move-ins and outs; hospitalizations, service level changes.

· Under the DON's direction, send letters to family/resident notifying them of service level changes.

· Maintain logs for required staff training and coordinate with HR and Campus Director, as necessary

· Attend and participate in new-hire orientation, workshops/seminars, and mandatory in-service trainings.

· Train oncoming staff and monitor and ensure compliance with training and education requirements for current staff.

· Participate as a team member in meetings, task forces, and quality improvement efforts. Maintain minutes of meetings/files as necessary.

· Prepare for new admission: Inform nursing service staff of new admissions, their expected time of arrival, room assignment, etc. Prepare chart with all necessary paperwork.

· Complete necessary medical and administrative records/documentation upon a resident's admission,
