Fairview Skilled Rehab & Nursing Home is looking for warm, reliable, and team-oriented CNAs to join our growing care team across all shifts. We serve residents ages 18 and up in a fast-paced rehab and long-term care setting. Our CNAs play a key role in supporting residents with dignity, patience, and compassion while partnering with our nurses, therapists, and interdisciplinary team.

Key Responsibilities

Resident Care & Support

• Provide hands-on assistance with ADLs, including bathing, grooming, dressing, toileting, transferring, and ambulation.
• Deliver high-quality care with respect, kindness, and sensitivity to each resident's needs.
• Assist residents with meals, hydration, and nutritional support.
• Check and monitor vital signs and promptly report any changes to the charge nurse.
• Support restorative nursing programs and therapy carryover as directed.

Documentation & Safety

• Accurately document all care provided in the electronic charting system.
• Follow all infection control protocols and DOH regulations.
• Ensure resident rooms and units are clean, safe, and well-organized.
• Report incidents, concerns, or equipment issues immediately.

Communication & Teamwork

• Collaborate closely with nurses, rehab, recreation, and social work teams.
• Engage respectfully with residents, families, and visitors, maintaining Fairview's warm and welcoming culture.
• Participate in handoff reports and contribute to a positive, solutions-focused work environment.

Qualifications

• Current NYS Certified Nursing Assistant (CNA) license required.
• Strong communication skills, patience, and the ability to work with diverse resident needs.
• Ability to lift, move, and assist residents safely.
• Reliable, punctual, and able to work independently as well as part of a team.
• Experience in skilled rehab or long-term care preferred but not required — we provide training and support.
