Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong background in patient care, with experience across various medical settings including long-term care, assisted living, hospital environments, and outpatient clinics. The LPN will play a vital role in providing high-quality nursing services, supporting patient recovery, and ensuring compliance with healthcare standards such as HIPAA and NCQA. Proficiency with EMR/EHR systems like Epic, Cerner, and eClinicalWorks is highly desirable. This position offers an opportunity to work in diverse clinical settings, including geriatrics, pediatrics, acute care, and specialty areas such as dialysis, hospice, and infectious disease care.

Responsibilities

  • Conduct patient assessments including vital signs, physical examinations, and health history documentation
  • Administer medications and injections accurately following physician orders
  • Perform IV insertions, catheterizations, and sterile procedures with aseptic technique
  • Manage airway support including ventilator care and airway management techniques
  • Assist with diagnostic evaluations such as blood draws (phlebotomy), specimen collection/processing, and laboratory testing
  • Provide wound care including suturing and dressing changes
  • Support patients with activities of daily living across various settings such as memory care, dementia care, and behavioral health units
  • Document all patient interactions thoroughly using EMR/EHR systems like Epic or Cerner while maintaining HIPAA compliance
  • Educate patients and families on health management plans including chronic disease management (e.g., Alzheimer's care, pain management)
  • Collaborate with interdisciplinary teams for discharge planning, case management, and post-acute care coordination
  • Respond to urgent situations in emergency or urgent care settings by applying basic life support (BLS) skills and advanced physiology knowledge

Requirements

  • Valid Licensed Practical Nurse license in the state of practice
  • Proven experience in diverse clinical environments such as hospital medicine, ICU/CCU/PICU/Neonatal ventilator units, outpatient clinics, or home & community care settings
  • Familiarity with ICD-9/10 coding systems for accurate documentation and billing purposes
  • Experience with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks preferred
  • Knowledge of infection control protocols including sterile processing and aseptic techniques
  • Ability to perform venipuncture (venipuncture), phlebotomy, catheter care, tube feeding administration, and medication administration with precision
  • Strong understanding of anatomy and physiology relevant to patient assessment and diagnostic evaluation
  • Excellent communication skills for working effectively with patients of all ages—including children—and multidisciplinary teams
  • Ability to work in high-pressure environments such as emergency rooms or trauma centers (Level I & II Trauma Center experience advantageous)
  • Willingness to adhere to strict safety standards including infection control practices and HIPAA regulations
